A hospital stay often requires additional services and care. That’s why our team of dedicated social workers assists patients and family members with financial, social, and emotional issues that may arise during or after hospitalization. Our social workers work with patients and families to help them deal with long-term illnesses, rehabilitation and continuity of care transitions.

Case Management
The Case Management Department offers assistance to in-patients who require help with their discharge plans or whose living arrangement may interfere with their care or recovery. The Case Management department is made up of nurses, social workers, and discharge planners.
