Multi-Specialty Clinic
Transition Safely from Hospital to Home
For patients, the transition from hospital stay to home is a critical time in their healing cycle – especially for those who are recovering from complex and chronic conditions. To help smooth this process and bridge the gap, Sherman Oaks Hospital offers a Multi-Specialty Clinic on its medical campus. Now, your hospital clinician can extend the continuity of care between your hospital stay and your follow-up Primary Care appointment by arranging a clinic visit within the first seven days of discharge to continue to assess your ongoing medical needs throughout your recuperation.
Did you know? Almost 60% of hospitalized Medicare patients did not have contact with a physician within two weeks following discharge. For patients with ongoing medical needs, misconnections are major contributors to hospital readmissions.1
